Health: I think you’re not quite reading the Rambam properly. You are seeing that the Rambam says that Ovdei A”Z are included in Kofrim because they are Kofrim (you are giving the word “Kofrim” a title and a Halachic status, which I don’t think exists). I read the Rambam as saying that Ovdei A”Z are included in those who deny HKBH. There are many types of Kofrim, and sometimes we have to relate to some of them differently.
