Does the reason one should not remove two garments simultaneously also apply to removing a shirt and undershirt simultaneously? Does it preclude putting ON two garments simultaneously?

Sure it applies. The reason is not a Halachic one, it is Al Pi Ari.

The problem is putting on two garments at the same time. That is kasha l’shikcha. Taking them off is not a problem.

DH: The way I learnt it was the problem with taking if off simultaneously.
why are you not supposed to take off two things at the same time? why are you not supposed to put two garments on the same time?

adorable: Putting on two garments at the same time causes forgetfulness. This is a problem for men who are not allowed to purposely forget their learning and so have to avoid things that cause forgetfulness.
im pretty sure kitzur sa mentions about taking off multiple garments.
