Most extended warranties are marketed by third-parties and generally are not cost-effective in terms of coverage. The only ones that MAY be a good option are those offered by the manufacturer and those are generally limited to vehicles serviced at their dealerships for the full term of the initial warranty. If you go online, you will find a gazillion complaints from those who purchased what they thought would be comprehensive coverage (like their initial warranty) only to discover most claims are denied for incomprehensible reasons and/or cover only non-OEM parts and impose obscene labor charges.
