Two years ago I was visiting a relative in the hospital when nurses came in to administer an ordered dose of medication. I was then treated to a conversation between the two as to whether or not .5 and the .50 that was written on the orders was the same thing.
Though this story was innocuous my grandfather always checked the meds he was given. He told me that once he caught a decimal mistake. He was about to receive 10x the ordered dose and he was told that if he had not caught the mistake the drug would in all likelihood have killed him.
