One mistake that I was told is that some people do not realize they are not supposed to talk during kaddish. When hearing kaddish, one is supposed to stop, answer amein and say the yehei shmei rabba… at the appropriate time. Someone told me that there is a place somewhere in E”Y that has carved into stone or something that one who talks while kaddish is being said, has lost his chelek in Olam Haba, because kaddish is the ultimate affirmation of gedulas Hashem, and to talk at that time is kivyachol like spitting in His Face. I don’t know if any of this is halacha, but it really makes sense that it is an abomibale lack of derech eretz for hashem to talk during this or any part of davening.
