That can actually happen if someone decides that something is really just DE without asking.

And is more likely to happen to someone who followed the OU’s advice and asked every 3 months, and it changed within the 3 months. (And presumably has a spreadsheet with each item he asked about and the last time he asked, along with the past 50 years worth of OU emails and product alerts, which he consults each time he goes to the store like a good little O-U jew.)
