I’ll leave it to others as to which rabbonim/poskim are most knowledgeable and responsive on these end-of-life matters but having recently dealt with the issues, I can only wish you have chizuk in whatever decision you make. Clearly the toughest decision we make in our lives and having emunah gives one the strength to follow through.
