A doctor cannot pasken that it has the halachik status of sakanas nefashos. I had family members who had chronic illnesses and each fast they would speak to the doctor and then relay the doctors words to the rav for a psak. There were times when the rav spoke to the doctor directly. Every time you bring this up it surprises me.
