Of course they should, but what if they don’t want to?
As the Doc or other practioner -do you say too bad on you -No goodies (meds) for you or do you prescribe meds anyway? In this country you can’t force a patient to do the right thing. So if s/he won’t go for therapy, you can’t be unethical & withold the meds that you know s/he needs!
