This reminds me of what Rav Aron Sloveichik zt”l once said about this issue.
What’s the difference between a Shaygetz and a Goy Gamur? A Shaygetz is a true non-Jew while a Goy Gamur is a frum Jew who the one calling this name disagrees with!
If a nickname that bothers the person is ossur,kal vachomer an outright insult!

The actual Issur is to give someone a name like shorty, funny face, Zalman the knucklehead and so on.
A name that sticks to him/her is a lot worse than any particular insult. A name puts a person into a category and it stays — other people pick it up.
What’s the Halacha regarding calling someone Joseph, though?
